Free medicine record template
Your medicine book has run out of pages, or you would rather not buy another one. This sheet carries every field an inspector checks. Print it, punch it, put it in the folder.
Before you print
- Set your printer to A4, portrait. The left margin is wide on purpose so a hole punch misses the writing.
- Print a stack. Records for food-producing animals have to be kept for five years, so most farms get through more sheets than they expect.
- The CSV opens in Excel, Google Sheets and Numbers. It has the same columns, so you can start on paper and move across later without re-keying the headings.
Filling it in
Clear on is the column people leave blank and then regret. Work it out when you write the treatment down, not on the morning the lorry is booked. Count from the day after the last dose unless the datasheet says otherwise, and if you are unsure, ask your vet rather than guess.
Keep your purchase receipts with these sheets. The two have to reconcile: if more product came in than the treatments account for going out, that gap is what gets picked up.

Withdrawal periods are product-specific and change. Always check the datasheet for the product and species you are treating, and follow your vet’s direction where a medicine is used under the cascade. This sheet is a record, not veterinary advice.

Three of the columns above are jobs you do in your head, and they are the three that go wrong. Agrianta does them for you.
- The “clear on” date
- Scan the bottle and the withdrawal comes off the VMD register. The date every treated animal is clear is worked out and counted down for you, so nothing loads that shouldn’t.
- Reconciling against purchases
- Stock comes down as treatments are recorded. The gap between what came in and what was used, the thing an inspector looks for, is on the screen rather than in a shoebox of receipts.
- Five years of paper
- Records are kept for you and print on demand, which is what settles the paper-or-digital argument at the gate. Recorded at the crush from a phone, with no signal needed.
Keep using the paper sheet if it works for you. It is a legal record either way, and it is free.
